Semax Dosage Guide

Semax is a synthetic analog of the adrenocorticotropic hormone (ACTH) fragment 4-10, with the sequence Met-Glu-His-Phe-Pro-Gly-Pro. It was developed at the Institute of Molecular Genetics of the Russian Academy of Sciences and has been approved in Russia since 2011 for the treatment of stroke, cognitive disorders, and peptic ulcers.

Research demonstrates that Semax increases brain-derived neurotrophic factor (BDNF) expression, enhances attention and memory, provides neuroprotection against oxidative stress, and modulates serotonergic and dopaminergic systems. Unlike ACTH, Semax does not stimulate cortisol production or affect the adrenal glands.

Semax is primarily administered intranasally. Dosing varies by variant:

The 0.1% nasal solution (1 mg/mL) is the standard formulation approved in Russia. The N-Acetyl variant (NASA) is reported to be 50–100x more potent and is dosed accordingly lower. Standard daily totals range from 400–1800 mcg for regular Semax.

Semax is administered 2–3 times daily via intranasal spray or drops:

• First dose: Morning, for cognitive enhancement throughout the day.
• Second dose: Early afternoon, to sustain effects.
• Third dose (optional): Late afternoon. Avoid evening dosing as it may interfere with sleep in some subjects.

Cycling: Russian medical protocols typically prescribe 10–14 day courses, with breaks of equal length. Research protocols may extend to 30 days. N-Acetyl Semax is typically used in shorter cycles (10–14 days) due to its greater potency. Semax does not appear to cause tolerance or dependence.

Semax is most commonly supplied as a ready-to-use nasal spray. For lyophilized powder:

• Step 1: Add sterile or bacteriostatic water to reconstitute. For a 25 mg vial, add 2.5 mL for 10 mg/mL.
• Step 2: Gently swirl to dissolve.
• Step 3: Transfer to a sterile nasal spray bottle (120 mcL per spray is standard).
• Step 4: Store at 2–8°C. Use within 14 days.

Intranasal administration: Tilt head slightly back. Insert nozzle into one nostril and spray. Alternate nostrils. Sniff gently — do not inhale forcefully. Semax can also be administered subcutaneously at similar doses if nasal spray is unavailable.

• 100–200 mcg/dose: Well-tolerated. Occasional nasal irritation from intranasal delivery.
• 200–600 mcg/dose: Possible mild headache, nasal dryness, or irritability in some subjects. Enhanced alertness may affect sleep if dosed too late.
• 600–1000 mcg/dose: Increased risk of overstimulation, anxiety-like symptoms in sensitive subjects, and more pronounced nasal irritation.

Semax has an excellent safety record in Russia where it has been used clinically for over a decade with minimal reported adverse effects.

Semax is approved in Russia and several CIS countries but is not FDA-approved in the United States. It is available for research purposes in most jurisdictions.

Semax should not be used in subjects with acute psychotic states or seizure disorders without medical supervision. While it does not affect cortisol, subjects with ACTH-related conditions should exercise caution. Consult a healthcare provider before use.
