Overview
Hexarelin (examorelin) is a synthetic hexapeptide growth hormone secretagogue that acts on the ghrelin/GHS receptor (GHSR) in the pituitary gland. It is considered one of the strongest GH-releasing peptides available, producing a more robust GH pulse than GHRP-6 or GHRP-2, though it does not significantly increase appetite like those peptides.
Developed in the 1990s, hexarelin reached Phase II clinical trials for GH deficiency and cardiac protection before development was discontinued. Research has shown it has direct cardioprotective effects independent of GH release, acting on cardiac GHS receptors to protect against ischemia-reperfusion injury. A significant limitation is rapid desensitization — the GH response diminishes substantially with continuous use over 4-8 weeks, requiring cycling.
